The Standard Survey Language template is part of PHSA's new survey review process, which clarifies the few conditions when privacy review is required for non-research projects (e.g. quality improvement).
Getting the introductory wording right for your survey is important for several reasons. It helps readers understand the purpose of the survey, how the information they provide will be used, and decide whether they would like to participate. To comply with the BC Freedom of Information and Protection of Privacy Act (FIPPA) and PHSA’s policies, there are key elements your introductory wording must include.
The language in the template, which has been reviewed by PHSA legal, must be used verbatim in the introductory wording of your survey.
If you are conducting a survey for purposes other than research, then you will need to read the criteria listed on our Conducting Surveys page to determine if your survey requires additional review.

criteria apply to your survey, then you can simply use the Standard Survey Language template and proceed with your survey.
If any of the criteria apply to your survey, you must complete and submit a Privacy Compliance Survey Review Checklist and submit it to PHSA’s Information Access & Privacy (IAP) or the BCCDC Privacy Office (where applicable) for review.

If you are conducting a survey for research purposes, you can use the Standard Survey Language template. Please note, however, that all surveys used for research must be submitted to a Research Ethics Board (REB) for review and approval, and must use the final REB approved language.
If you need help filling out the template or would like your survey reviewed, please email PHSA's Research Privacy Advisor and she will review your submission within 3 business days upon receipt. Please note that the Research Privacy Advisor can help you to adjust the wording to meet the needs of your participants if required. You do not need to submit a survey intake form.
